Survey options in myCourses
The optional settings available for Surveys are very similar to the options available for Quizzes (see Create and edit a quiz in myCourses) with the exception of the features explained below.
Properties
On the Properties tab of a survey, you can enable instant feedback, make the survey anonymous, or change the submission message at the end of the survey. As with Quizzes, you can also add a Category, Description, Footer, and/or Rubric.
General
Feedback: If you select this option and entered feedback for any questions, respondents will see a word balloon next to the responses for which you entered feedback. See Add hints and feedback to questions for more information.
Anonymous: This will prevent you from seeing who gave what responses on the survey.
If your survey already has responses, you cannot make it anonymous.
If the Anonymous checkbox is selected when you save the survey, you will receive a pop-up window asking if you want to confirm.
After you click the Yes button, you will not be able to change this option to make the survey non-anonymous.
You cannot view individual survey attempts for anonymous surveys. You can only view the overall results.
Although you can add Rubrics to an anonymous survey, you cannot use them to evaluate the attempts.
You cannot attach Release Conditions to an anonymous survey or create Release Conditions based on the completion of an anonymous survey.
Survey Questions
Under Survey Questions, you can access the Branching Wizard. As you do with Quizzes, click the Add/Edit Questions button to Create questions or Import questions into your survey. However, unlike other assessment types, you can add Likert Questions to surveys. Another difference is that the Edit Values button on a survey only allows you to make questions Mandatory or not, because there are no point values or grading options for questions within a survey.

Description / Submission Messages
Submission Message: Enter a custom message that will appear to respondents after they submit the survey. If you do not enter a message here, the Default Submission Message “You have successfully submitted the survey” will appear.
Restrictions
On the Restrictions tab of a survey, you can choose the number of times users will be allowed to take the survey or give them one editable attempt. Like Quizzes, you can also set Availability options. Release Conditions are not available for anonymous surveys.
User Responses
Attempts Allowed: Select one of the following options from the drop-down list:
Unlimited: Allows users to take the survey as many times as they want.
Single attempt that is editable: Allows users a single survey attempt, but they can re-access and edit their responses as long as the survey is still available. Editable surveys will appear to students with an Editable icon beside their name.
Editable surveys may not be desirable because if students click on an Editable survey after they have already submitted it and then exit the survey without clicking the Submit button again, their survey attempt will be listed as "in progress" and will not count in the Results.
Limited: Enter a Number of Attempts in the box that appears for how many times users will be allowed to take the survey.
Reports Setup
On the Reports Setup tab, you can create reports for the survey and release them to instructors and/or students in your course. See Viewing survey results for more information.
